^Pretty sure it depends on the lever. Some levers the cable will come out along the underside of the bar and goes under the tape, some (I guess higher end levers) will run the cable through the bars. If yours goes through the bars, you will of course need a hole for the cable to exit.

Almost definitely no. If you need a hole, it will alread have a hole.

cable routing is all down to the bars, if theres two holes near the stem, run it internally, if not go under the tape
